Previous conditions: Type 2 diabetes since 2009 - mainly treated with metformin, reasonably under control.
Presentaion in March 2020 - back, groin, pelvis pain & weight loss first noted in August 2019 but significantly more painful with physiotherapy, November 2019 - January 2020.
Referred to hospital in view of PSA of 520 [and some other high readings]. No biopsy taken - in view of PSA level - with Firmagon/Degarelix injections x2 administered on diagnosis. Scans followed over next month or so. Oncologist recommended chemo but changed her mind with onset of pandemic and availability of Enzalutamide/Xtandi tablets. Commenced Xtandi in late June 2020 with PSA of 3.9 [reduced from 520 by degrarelix]. PSA continued to move down; lowest reading was 0.13 in November 2021.
In 2022 PSA has been creeping up, with July 22 reading of 2.2. Scans [bone & CT] have been taken in early 2022 and again in August. Early 2022 scans indicate some minor progression but inconclusive. August 2022 scan results awaited.
PSA is still undetectable after more than 14 years. All is good.

Yesterday (20 February 2026) I attended Duncan's funeral in Essex, UK. Duncan was my 'cancer buddy' for the last 4 years. We both attended 2 support groups here in the UK. These groups were:
- 'ProActive - the prostate cancer support group covering East Herts and West Essex' which meets in Epping, Essex and is open to anyone.
- 'Living Well with Advanced Prostate Cancer' which is a nationwide group that meets as a Zoom call and is by invitation being restricted to stage 4 sufferers.
I understand that Duncan had not updated his journey for over 2 years. I am sending this email in case it might be useful to update it and record the fact that he has passed. Duncan put up a brave fight, his treatment included Lutetium-177 which is still fairly rare. Despite this, I think the last time that he told me about his PSA the reading was 180 in Summer 2025. He died on 25 January 2026 peacefully at home with nurses from a local hospice visiting daily to administer palliative care.
